Job Description:

A company is looking for a Part-Time RN Disease Management Nurse (Weekends Required).
Key Responsibilities

Coordinate wellness and disease management for targeted members, focusing on various chronic conditions
Develop individual care plans based on claims data and ensure follow-up with patients
Assist patients in obtaining necessary healthcare services and document interventions while maintaining privacy

Required Qualifications

Graduation from an accredited RN program and possession of a current United States RN license
Prior experience in case management or wellness program coordination
Ability to work independently with excellent judgment
Proficient in Microsoft Office Suite (Excel, Word, PowerPoint, Outlook)
Strong organizational and time management skills
